Program Coordinator – Housing First ProgramPosition Type: Contract, full-time, 35 hours per week. Contract ends March 31, 2028. Salary: $63,345 – $76,704 per annum (based on experience). Evening hours and weekends may be required. Closing Date: June 5, 2026.LocationThe Program Coordinator will work from the Aurora Office at 15150 Yonge St, Aurora ON L4G 1M2 on assigned days of in office work and will perform community/client visits with regular travel in York Region & South Simcoe.Job ProfileThe Program Coordinator collaborates with the Manager to coordinate mental health, housing, and financial support services, manage client referrals, support immediate financial requests, coordinate with third parties (private landlords, moving companies, insurance, cleaning services), and provide direct mental health support to housing clients.ResponsibilitiesProgram CoordinationMonitors incoming referrals, community outreach, and requests for workshops and community events.Assists with coordination of community events approved by the Manager.Consults with the Manager to determine program capacity for workshop and group requests.Receives and confirms new referrals, conducts initial needs assessment screenings, documents activity in CRMS and HIFIS, and forwards clients to appropriate services within Housing Programs.Monitors client waitlists and collaborates with Clinical Supervisors to assign clients to program staff as directed.Monitors financial requests, liaises with Finance team, tracks spending,



and reports on unique individuals served.Maintains monthly statistical records of services and prepares year‑end summaries of program activities.Maintains current knowledge of mental health and recovery practices and prepares reports for funders.Direct Service DeliveryDelivers mental health and case management supports to a caseload of clients.Conducts needs assessments, housing supports, supportive counselling, referrals, case coordination, service navigation, and individualized supports.Collaborates with other professionals to coordinate client treatment plans.Documents all client-facing services in CRMS, HIFIS, and manual tracker.Maintains familiarity with and access to formal and informal supports in CMHA York Region and the broader community.Applies recovery principles and empowerment‑oriented practices in work with consumers.Observes boundaries, engages in appropriate emotional regulation, refrains from dual relationships, maintains confidentiality, and employs self‑care strategies.Adheres to relevant provincial laws and regulations (Mental Health Act, Child and Family Services Act, Health Care Consent Act, Ontario Human Rights Code, Occupational Health and Safety Act).Demonstrates excellent diplomacy, decision‑making, problem‑solving, and teamwork skills.RequirementsDegree or diploma in health and social services, housing services, or related field.Current registration with OCSWSSW, OACYC,



or other regulatory body under the Regulated Health Professions Act.Second language (e.G., Cantonese, Mandarin, French, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu) reflecting the local community is an asset.Four or more years of experience in the mental health field; housing sector experience is an asset.Formal training in mental health, education, group work, program planning and evaluation, community facilitation, and development.Knowledge/skills/experience in counselling, case management, workshop/group development and delivery, and familiarity with solution‑focused brief therapy or other brief counselling approaches.Ability to apply recovery principles and empowerment‑oriented philosophies and practices.Ability to observe boundaries, regulate emotions, refrain from dual relationships, maintain confidentiality, and engage in self‑care strategies.Knowledge of relevant provincial laws and regulations.Extensive knowledge of supports and services in York Region, including formal and informal resources.Excellent diplomacy, decision‑making, problem‑solving, and teamwork skills.A valid Canadian driver’s license and minimum $1,000,000 third‑party liability insurance.A satisfactory and current Vulnerable Sector Screening.Working ConditionsOffice work, community settings including schools, meetings in clients’ homes, and travel with clients.
