OVERVIEW:

Reportingto the Northern Care Supervisor, the Northern Care Case Manager is responsiblefor providing in-depth Case Management services, ensuring quality assurancemeasures are followed, conducting licensing practices, offering guidance &support; for families, children, & communities in the Northern Care Program.Fifty percent (50%) of work time will be spent in Community on a rotation.

KEYRESPONSIBILITIES:

- Providing extensive CaseManagement Services to House Parents & homes in the Northern Care Program’sHouse Parents homes located in the 8 Southeast First Nations.
- The Case Manager isresponsible for planning for & overseeing day-to-day operations of thehomes to ensure the needs of the children placed in the home are being met.
- Overseeing licensing dutiesof Northern Care Program homes.
- Recruitment, screening,initial assessments, & interviews of potential House Parents.
- Ensuring proper checks& paperwork is completed for licensing.
- Conducting orientations forpotential/new House Parents.
- Completing Places of Safety& House Parents home studies for current House Parents.
- Completing annual reviews,addendums, & closures as needed for each home in a timely manner.
- Conducting House Parentremovals when needed.




- The Case Manager isresponsible for providing the oversight needed in the Northern Care Programhomes to ensure compliance with Foster Care Regulations & Standards.
- Working closely withcolleagues within the Northern Care Program & other SAI departments toensure successful service delivery.
- Establishing positive &healthy; relationships with & providing support to the Northern Care ProgramHouse Parents through regular communication, meetings, & home visits.
- Participating in crisisintervention / de-escalation, mediation, & de-briefing with House Parents.
- Support House Parents inunderstanding & complying with Program expectations & their roles, aswell as the completion of important documentation (incident reports, monthlyreports, etc.).
- Following up &documenting; concerns brought forward.
- Developing House ParentCare Plans.
- Consistent regular traveland overnight stays to First Nation Communities to accommodate licensing, HouseParent meetings and meet with local CFS. Travel by plane, boat, helicopter,etc. Regular travel is a requirement of this position to build relationshipswith House Parents, supports, families and community. Fifty percent (50%) ofwork time is spent in Community on rotation.
- Be informed ofCommunity-based events, promoting & supporting community engagement byensuring children & families attend local community events.
- Providing regular support,guidance, & mentorship to the Community Coordinators & Community staffin our 8 Southeast First Nation Communities.
- Collaborating withCommunity Coordinator & Support Worker Coordinator to develop &maintain; consistent schedule of Respite & Support Worker shifts.
- Regular home visits at eachNorthern Care Program House Parent home.
- Collaborating with SECFSDirect Services Workers & Supervisors.
- Planning & preparingfor child placements & discharges.




- Ensure outcomes,assessments, & service plans are current & up to date.
- Updating & keepingdetailed case notes for all homes, including clear documentation.
- Developing, implementing,& monitoring maintenance of Northern Care Program Home Care binders.
- Review monthly reports& bi-weekly file records for each home, monitor for concerns or areas ofimprovement.
- Review program memos &directives; with House Parents/Community Coordinators.
- Communicate to NorthernCare Program Supervisor the observed training needs for House Parents, Support& Respite Workers.
- Regular travel to Communityand overnight stays in Community is a requirement of this position.
- Ability to lift and carryitems up to a minimum of 50lbs

QUALIFICATIONS:

- Bachelor of Social Work (BSW) or equivalent ispreferred.
- A minimum of two (2) years experience in front lineCase Management.
- Knowledge of Community Based resources & how toaccess them.
- Knowledge in the areas of addictions, attachment,mental health & trauma & how it impacts families.
- Knowledge of the Child & Family Services (CFS)Sector & Foster Care Legislation.
- Demonstrate knowledge and/or understanding ofIndigenous perspectives, First Nations culture & traditional philosophies.
- Strong assessment skills & experience conductinginvestigations including gathering an analyzing information to implementappropriate actions.
- Experience developing & supporting a strong,respectful team-based approach to service delivery.
- Demonstrate knowledgeand/or understanding of First Nations perspectives, culture, and philosophies.
- Mustmaintain a high level of confidentiality.
- Ability to work in a fast-paced environment.
- Must be resourceful with a high attention to detail.
- Ability to prioritize tasks and effectively managemultiple priorities.
- Must have a clear child and adult abuse registrychecks, criminal record check with vulnerable sector search and Prior Contact Check,required.
- Valid driver’s license and a reliable vehicle with asatisfactory driver abstract, required.
