Outreach Nurse – Pharmacy & Community Outreach

About Us

We are a community-focused pharmacy committed to improving access to medications and health services for individuals who may face barriers to traditional healthcare. We are looking for a compassionate, reliable, and community-minded Outreach Nurse to join our team.

This position will support our pharmacy’s medication delivery services while providing outreach and nursing support to individuals experiencing homelessness and those living with mental health and substance use challenges.

Position Overview The Outreach Nurse will work both independently and as part of our pharmacy and community care team. The successful candidate will help ensure patients receive their medications safely and reliably while building trusting relationships with individuals who may experience barriers to accessing healthcare.

The role involves community outreach, medication-related support, health education, coordination with healthcare providers and community organizations, and helping connect patients with appropriate health and social services.

Key Responsibilities

Medication Delivery & Patient Support

- Deliver medications to patients in the community in a safe, professional, and timely manner.
- Support medication adherence and provide education regarding medications within the nurse’s scope of practice.
- Identify concerns related to medication use, adherence, or changes in a patient’s condition and communicate appropriately with the pharmacy and healthcare team.
- Maintain appropriate documentation and records related to patient interactions and deliveries.
- Protect patient confidentiality and comply with all applicable privacy and professional requirements.

Community Outreach





- Provide outreach services to individuals experiencing homelessness and individuals living with mental health and/or substance use challenges.
- Develop respectful, trusting relationships with clients and provide a non-judgmental, trauma-informed approach.
- Conduct basic health assessments and provide nursing support within the scope of practice.
- Provide health education, harm-reduction information, and assistance navigating healthcare resources.
- Help connect clients with physicians, mental health services, addiction services, shelters, social services, and other community resources.
- Follow up with clients when appropriate to support continuity of care.
- Work collaboratively with community agencies and healthcare professionals.

Team Collaboration

- Communicate effectively with pharmacists, pharmacy staff, physicians, nurses, social workers, and community service providers.
- Participate in care coordination and case discussions as appropriate.
- Recognize situations requiring escalation or referral to emergency or specialized services.
- Contribute to a positive, respectful, and client-centered workplace culture.

Qualifications
- Current registration as a Nurse in British Columbia with the BCCNM.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation, if required for community deliveries.
- Experience in community nursing,



outreach, mental health, substance use, primary care, public health, or a related setting is an asset.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently while also collaborating effectively with a multidisciplinary team.
- Comfortable working with individuals from diverse backgrounds and with complex social and healthcare needs.
- Demonstrated ability to maintain professional boundaries and patient confidentiality.
- Naloxone training and experience with harm-reduction services are considered assets.

Experience working with vulnerable populations is an asset, but we also value a willingness to learn, strong clinical judgment, and a genuine commitment to improving access to healthcare in the community.

The hours are generally going to be from 7.30am - 1.30pm but these be adjusted for the right candidate and we looking for any 5 out of the 7 days for coverage.

Pay: From $35.00 per hour Expected hours: 30.0 – 35.0 per week
