Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.Program Coordinator, Home Support Services (Part time)The incumbent is responsible to the Program Manager/Assistant Director of Home Support Services,



for the overall day to day coordination of Support Services to the communityResponsibilities:Coordinates the delivery of appropriate home support services to enable clients to age in placeCoordinates meal and transportation services for clientsProvides timely and professional client intervention to clients and familyMonitors overall health status and keeps up-to-date health records of clientsCoaches, develops and evaluates staff to ensure they meet their required performance standard.

Accurately schedules staff resources in accordance with clients' needs as reflected in the care planEnsures implementation of risk management policy and safe environment for clients and staffIn consultation with the Manager, develops and updates policies, procedures for the programCoordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteersMaintains up to date and accurate files, records and statistics on the programEnsures efficient and responsible use of resources and maintains accurate record of fees and expendituresProvides emergency response to emergency calls from staff and clientsEffectively promotes the home support services to clients of the building/neighbourhoodKeeps up-to-date with developments and best practices in the fieldOther duties as assignedQualifications:Bachelor's degree in social services, health care, activation, or equivalentMinimum 2 years experience in social services or health care,
