Key Responsibilities:
• Drive strategic leadership within Continuing Care initiatives

Requirements:
• Bachelor's Degree; a Master's is advantageous
• Extensive experience in leadership roles in health systems

• Proven ability to influence and secure stakeholder buy-in
• Capacity for navigating sensitive operational issues
