In the Seniors Services and Long-Term Care Division, your mission will be to assist elderly clients in accessing essential services that enhance their quality of life. This position requires a blend of assessment abilities, documentation skills, and collaboration with community organizations to fulfill the needs of vulnerable individuals. Key Responsibilities:
Assess functional status and eligibility of clients
Document client information and create comprehensive case files
Liaise with service providers for coordinated care
Manage a caseload with follow-ups and re-assessments
Respond to client needs and emergencies promptly Requirements:
Degree or Diploma in Social Services or relevant experience
Considerable experience in community services
Solid interpersonal and organizational skills
