This part-time role supports residents while implementing tailored care plans in a supportive workplace. You'll report directly to the Director of Care, assessing residents’ clinical needs and ensuring effective care plans are executed. Solid communication and care coordination skills are vital, as you will collaborate with health professionals and families to enhance resident care experiences. Key Responsibilities:
Implement and evaluate resident care/service plans
Assist in resident care conferences and facilitate transitions
Document care and medication administration accurately
Educate residents and families on health management
Respond to medical emergencies and significant changes in condition Requirements:
Valid LPN registration in Alberta
Experience in senior care preferred
Robust clinical knowledge and customer focus
Basic Cardiac Life Support certification needed
