Make a difference as a Client Support Worker with WoodGreen in Toronto. This full-time, onsite position focuses on enhancing the lives of seniors overcoming homelessness through supportive engagement and individualized assistance.

As an integral member of the Street to Home Supportive Housing program, you will implement care plans, provide life skills training, and navigate health and social services for clients with mental health and substance use issues. Your role will foster positive relationships and collaborative support within a community-focused workplace.

Key Responsibilities:
• Develop respectful relationships and emotional support
• Assist with care planning and community resource navigation
• Participate in the intake and onboarding for new clients




• Monitor client well-being and adjust care plans accordingly
• Promote participation in community activities and stabilization

Requirements:
• Social Service Worker diploma or similar qualification
• 2+ years working with older adults experiencing homelessness
• Experience with marginalized, high-risk clients
• Proven skills in communication and crisis intervention
• Familiarity with local community resources
