Vancouver Coastal Health (VCH) invites applications for a Primary Care/Integrated Care Team Nurse (RN/RPN) at Lily's Community Health Centre in Vancouver, BC. You will work in an integrated team with Primary Care, Mental Health and Home Health to plan and coordinate patient care.
The position requires current RN or RPN registration with BCCNM, at least 1 year of related experience in a community setting, and a valid BC driver's license.
