The Winnipeg Regional Health Authority is seeking a Case Coordinator for Home Care Access Transcona to assess needs for home or personal care homes and coordinate services for elderly or infirm adults. The role requires planning, communication with agencies, and caseload management.

Two years of related community experience is preferred; a registered health skilled degree is required. This maternity term is in-person with a 7.25-hour day, plus evenings and weekends.
