This permanent, full time role within the Toronto Central South Regional Cancer Program empowers Indigenous patients, helping them overcome barriers and access necessary resources. You will collaborate closely with healthcare providers and community organizations to enhance patient experiences along their cancer treatment pathways. Your expertise in cultural protocols and community engagement will be crucial.

Key Responsibilities:
• Provide advocacy and coordination for Indigenous patients and families
• Facilitate culturally appropriate support and access to community resources




• Participate in case discussions and care planning
• Identify and address barriers affecting Indigenous patient care
• Educate and support healthcare staff in cultural competence

Requirements:
• Preferred degree in Social Work, Nursing, or a related area
• Minimum 3 years of relevant experience
• Strong knowledge of Indigenous cultures and healing practices
• Excellent interpersonal communication and decision-making skills
• Ability to travel across the Toronto Central region
