Key Responsibilities:
• Conduct medication reconciliation and pharmacotherapy assessments

• Participate in educational activities for pharmacy trainees
• Collaborate with interdisciplinary teams effectively

Requirements:
• Bachelor of Pharmacy or Doctor of Pharmacy required
• Active registration with the Ontario College of Pharmacists
• Hospital pharmacy experience is preferred
• Clinical experience with a rehabilitation focus beneficial
• Excellent communication and analytical skills
