Help individuals thrive as a Connection Coordinator with House of Friendship’s HART Hub in Cambridge. This role enables you to provide critical support to those experiencing homelessness and addiction challenges.

As a full time, permanent team member, you will embrace a person-centered approach to connect community members with resources that enhance their wellness. Work collaboratively with professionals and participants to develop meaningful support strategies that foster recovery and stability.

Key Responsibilities:
• Engage participants to build trust and facilitate wellness
• Develop individualized support plans tailored to needs
• Provide comprehensive support linking to resources
• Assist with treatment planning and aftercare support




• Maintain confidentiality and documentation compliance

Requirements:
• Minimum College Diploma or University Degree in a related field
• 2 years of experience in case planning/management
• Knowledge of addiction recovery and harm reduction
• Experience with populations facing socioeconomic challenges
• G class driver’s license for travel in Waterloo Region
