As a member of the pharmaceutical care team, you’ll collaborate closely with patients and healthcare providers while managing complex medication regimens. This full-time position requires a robust clinical background and the ability to facilitate communication within interdisciplinary teams. You will play a key role in patient assessments and developing therapeutic plans tailored to individual needs.

Key Responsibilities:
• Identify drug therapy problems with patient collaboration
• Provide documentation and reporting on medication management




• Educate patients and teams on drug-related issues
• Support process improvements in patient safety
• Conduct mentoring sessions for pharmacy students and residents

Requirements:
• Registration with College of Pharmacists of BC
• Two years of patient care experience required
• Valid Class 5 driver’s license necessary
• Proven communication and organizational skills
• Ability to integrate evidence into practice decisions
