As the Community Inclusion Program Coordinator, you will manage daily operations, oversee employee performance, and engage with individuals and their families. Your work will focus on creating a community-based resource environment that promotes independence and personal growth. Collaboration with the Home Share Coordinator’s network team will be vital to your success.

Key Responsibilities:
• Monitor compliance with CARF standards
• Create community programs tailored to individual needs
• Support recruitment and management of employees




• Coordinate after-hours responses for incidents
• Ensure all records and payroll details are accurate

Requirements:
• Minimum three years of experience in Community Inclusion
• Solid skills in Microsoft applications and Share Vision
• Unrestricted Class 5 driver’s license
• Current First-Aid and CPR Level C Certification
• Skills necessary for confidentiality practices

Lead initiatives in community service that promote inclusivity and support at PCCR.
