Fraser Health is hiring a Relief Full Time Indigenous Health Liaison Social Worker in Langley, British Columbia. This position focuses on delivering culturally relevant services to Indigenous clients and enhancing their access to health care resources.

You will work within a multi-disciplinary team to provide essential counselling and support services to First Nations, Metis, and Inuit populations. Responsibilities include conducting client assessments, advocating for their needs, and facilitating connections with healthcare providers. The role is vital in promoting holistic health within Indigenous communities.

Key Responsibilities:
• Facilitate service delivery between clients and health providers
• Conduct detailed assessments of client health and well-being




• Plan and promote health education initiatives for Indigenous families
• Act as a representative of Indigenous Health in the community
• Maintain records and documentation to comply with standards

Requirements:
• Bachelor’s degree in Social Work with two years of relevant experience
• Experience with Indigenous populations required
• Full registration with BC College of Social Workers
• Valid BC Driver's Licence with access to a vehicle
• In-depth knowledge of Indigenous health protocols
