Bridge Clinic Overview: The Bridge Clinic is a multidisciplinary primary health care team that provides an access point to coordinated care for adults aged 17 years and older who do not have a regular family physician or nurse practitioner. The clinic was established to bridge gaps in care while patients work toward connection with a permanent primary care provider.

The Bridge

Clinic provides team-based care for family medicine-sensitive conditions and supports patients through assessment, treatment, follow-up, and care coordination. Care is provided until the patient’s health needs are stabilized and appropriate resources and support are in place to promote ongoing health and well‑being. As a primary health care clinic, the Bridge Clinic is not intended to provide emergent care.

Rather, the clinic’s role is to provide a temporary medical home, offering timely assessment, follow‑up and coordinated care for patients who do not have an established primary care provider.

The Bridge

Clinic team includes family physicians, nurse practitioners, registered nurses (RNA and RNB), CPAS coordinators, social work, pharmacy, and rotating specialists. All team members are expected to work collaboratively within a multidisciplinary model, participate in clinical consultation, and contribute to coordinated, patient‑centred care.

The Bridge Clinic Medical

Director will work collaboratively with the Bridge Clinic leadership team to provide clinical oversight and guidance, support day‑to‑day clinical and operational leadership, and provide mentorship to nurse practitioners and other members of the multidisciplinary team.

The Medical

Director will also play a key role in supporting complex case management and advancing clinical research, quality improvement (QI) and innovation initiatives. Working within a dyad leadership model alongside administrative leadership, the successful candidate will help foster a culture of evidence‑informed practice, interprofessional collaboration, continuous learning, clinical excellence, and patient‑centred care.

The Medical Director of the Saskatoon Bridge Clinic will report to the operational leadership team of the Bridge Clinic (Area Division Lead and Operational Manager/Director) The Medical Director of the Bridge Clinic will function collaboratively with the Bridge Clinic leadership and operational team members to:

- Participate in advisory committees that are related to the Bridge Clinic
- Represent and communicate project and operational issues/goals to operational and clinical leaders, clinicians,



and Ministry.
- Lead, promote and effectively articulate to frontline physicians and clinicians the vision of the Bridge Clinic programs/services and how the Bridge Clinic will influence and improve safe, quality client care.
- Develop coalitions, collaborate effectively with internal SHA stakeholders, and draw on expertise in the community, to help establish the multi‑disciplinary clinical care model of the Bridge Clinic.
- Work collaboratively to establish clinical care standards of practice within the Bridge Clinic and develop protocols related to the same.
- Work collaboratively to identify required training for members of the multidisciplinary team.
- Build, through engagement, collaboration and support, a network of committed and motivated physicians and clinicians to work at the Bridge Clinic.
- Create and sustain a workplace environment that fosters the application of evidence informed practice, compliance with regulations and accreditation standards, utilization of clinical research, and management of risk and adverse events in collaboration with corporate resources.
- Assist with implementation of clinical services and service/program evaluation.
- Assist in reviewing any complaints, as requested, concerning the provision of clinical care by the Bridge Clinic physician team.
- Review patient care reports and audits by following up on reviews, as required or requested by the SHA.
- Upon request of the SHA, participate in quality improvement and performance measurement initiatives and recommend measurement tools to the Director and Managers responsible for Bridge Clinic.
- Develop and demonstrate leadership capabilities and commitment to the culture of clients first continuous learning.
- Lead and engage with Bridge Clinic leadership, as well as internal stakeholders and community organizations to develop an environment of cultural competence, focused on Truth and Reconciliation.
- Other duties as mutually agreed between the Medical Director and the Authority.

Key Responsibilities & Accountabilities: 1.

Direct Clinical

Care (0.7 FTE):

- Provide comprehensive, full-scope family medicine care to patients served by the BRIDGE Clinic.
- Deliver compassionate, patient‑centered care in a multidisciplinary primary health care setting.
- Maintain accurate, timely clinical documentation and manage patient panels in accordance with professional standards. 2.

Clinical

Support & Mentorship (0.2 FTE):

- Participate in advisory committees that are related to the BRIDGE Clinic
- Provide ongoing clinical mentorship, chart reviews, and informal and formal consultation to Nurse Practitioners (NPs) and allied health professionals within the clinic team.




- Work in partnership with clinic administrative leadership to establish, document, and maintain high clinical standards, evidence‑based care protocols, and efficient patient flow/care transitions.
- Collaborate with administrative dyad partners to plan physician/NP clinical schedules and ensure appropriate, safe clinical coverage.
- Review patient care reports, clinical audits, and patient care complaints concerning clinic provision, recommending course corrections as required.
- Lead and engage with clinic staff and community stakeholders to cultivate an environment of cultural safety, inclusivity, and commitment to Truth and Reconciliation.
- Other duties as mutually agreed to between the Medical Director and the Authority 3. Research & Quality Improvement (0.1 FTE):
- Lead, participate in, and promote local clinic QI processes, utilizing patient‑first continuous learning methodologies and quality measurement tools.
- Facilitate the integration of evidence‑informed practice, compliance with regulations and accreditation standards, and initiative‑taking management of clinical risk.
- In collaboration with academic partners (including the University of Saskatchewan College of Medicine), support, conduct, or facilitate primary care clinical research and scholarly activities relevant to the clinic population. Job Qualifications - Licensed medical practitioner in positive standing with the College of Physicians and Surgeons of Saskatchewan (CPSS).
- Canadian Certified Physician Executive Credential would be an asset.
- Advanced education in Health Care Administration, Public Administration and/or Leadership Studies would be an asset.
- Participation in the Canadian Medical Association’s Physician Leadership Institute would be an asset.
- Additional training and/or education in leadership/management would be an asset.
- Experience, at an operational level, as a medical leader who has worked in partnership with administrative leaders to develop and lead innovative planning and programs in a complex, multi‑stakeholder environment.
- Has knowledge of and experience in guiding health quality improvement processes and initiatives.
- Valid Class 5 driver’s license.
