As part of a compassionate team, you will report to the Site Administrator, guiding care planning and admission processes for families and residents. Responsibilities include working with interdisciplinary partners to facilitate assessments, coordinate resident orientation, and promote a secure environment. Your role will also include quality initiatives and supportive training for staff.

Key Responsibilities:



• Provide individual counselling and therapeutic support • Support staff in responsive behaviour strategies • Coordinate resident admissions and tours promptly • Complete behavioral assessments for individuals • Collaborate in resident care conferences and meetings

Requirements: • Current registration with the Ontario College of Social Workers • Demonstrated skill within health care settings • Ability to lead and communicate effectively • Knowledge of dementia and long-term care practices • Experience with Behavioural Supports Ontario preferred
