This is a Regular Full-Time OpportunityType: ReplacementAn exciting opportunity as a RegularFull-Time Social Worker existsin the Women's College Academic Family Health Team (WCAFHT), reporting to the Executive Director, Environmental Health Clinic and WCAFHT. The Women's College Academic Family Health Team is one of the largest Family Practice Units within the Department of Family and Community Medicine with over 40 staff Physicians, interprofessional team, administrative and support staff.Summary of Duties, but not limited to:Provide patients and families with emotional support, coping strategies and linkage to community resources and support, including but not limited to financial assessment, social and financial assistance, options for disability support, respite care, and child care.Provide support, short-term and longitudinal psychosocial therapy and counseling through stages of women's treatment journey, from diagnosis to aftercare and survivorship.Conduct assessments and participate in the development of treatment plans with other team members.Coordinate referrals and facilitate access to other health care providers and organizations, while assisting patients and their families with the development of social and community support systems.Develop, implement and balance individual, group, same day, in person and virtual counselling and patient and family education sessions.Work collaboratively with all members of the health care team internal and external to the organization while ensuring continuity of care as patients transition across the continuum through appropriate discharge planning, patient education,



and coordination of services to community support and resources.Establishes a caring and therapeutic relationship with patients; demonstrates trust, respect, and honesty and provides counseling including crisis interventions as requiredAdvocates for patients by mobilizing resources and identifying and addressing gaps in service to meet supportive care needs of patientsIn collaboration with Professional Practice, Research, and Education departments initiatives and participates in clinical research and teaching and integrates research into practice to ensure best practice standards are maintained.Identify and lead quality improvement initiatives that will advance the work of the Centre and WCHAssist patients and families in overcoming barriers that have the potential to impede access to careSupport and coordinate patient care for marginalized and complex patient population while supporting and enabling strong community linkage to applicable resourcesSupport and champion quality improvement and best initiatives such as social prescribingAs a role model and champion, you will work to identify and integrate safe,



best practices into daily activities to foster the delivery of safe and exemplary care.The responsibilities described above are representative and are not to be construed as all-inclusive.QualificationsRegistration with the Ontario College of Social Workers and Social Service Workers is requiredMaster's degree in social work is requiredMinimum of 5 years' experience in a hospital, ambulatory setting is requiredAbility to form positive therapeutic alliance and to work independently and within a multidisciplinary teamUnderstanding of adult education strategies, teaching and learning principles, and advanced therapeutic communication techniquesAbility to develop collaborative working relationships with a wide variety of internal and external stakeholders including community organizations, and hospitalsExcellent critical thinking, problem solving, organization, and time management skillsWell-developed verbal and written communication, interpersonal, and teaching skillsProficient with Electronic Medical Record (PSS EMR), Microsoft Word, PowerPoint, and navigation of the InternetDemonstrated commitment to a patient/family centred care environmentEvidence of ongoing professional development, continuing education and leadership experienceDemonstrated valuable attendance record with the ability to maintain this same standardExperience working with marginalized and complex patient populationExperience with case management and community linkageProven attendance and punctualityProfessional behavior and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women's College HospitalThis position plays a critical role in acting as an advocate for safety and will demonstrate principles, practices and processes that will optimize a safe environment for all
