Through personalized OPTIONS, we collaborate with people and community partners to develop and offer creative personal and clinical services for individuals.Position will require:Diploma or degree in Social Work, Child and Youth Care, Developmental Services, Human Services, Psychology, Sociology, Education, Community Development, or a related fieldExperience supporting youth, families, people with developmental disabilities, and/or individuals with complex support needsKnowledge of transition planning, community development, service coordination, advocacy, and system navigationKnowledge of Developmental Services Ontario (DSO), developmental services, housing resources, mental health support, employment services, education systems, and community resourcesStrong interpersonal, communication, facilitation, advocacy, presentation, organizational, and problem-solving skillsExperience developing community partnerships and collaborating with multiple service providers and stakeholdersAbility to work independently while managing multiple priorities and deadlinesProficiency in Microsoft Office and electronic documentation systemsValid Ontario Driver's License, reliable vehicle, and appropriate insurance coverageCurrent First Aid and CPR certification or willingness to obtainFlexibility to work evenings and occasional weekends as requiredReporting to the Manager of Development and Engagement,



the Community Services Navigator will support transition-aged youth with developmental disabilities and their families as they prepare for and navigate the transition from children's services to adult developmental services. The successful candidate will provide information, advocacy, education, coordination, and support while promoting person-centered planning, informed choice, community inclusion, and independence.The Community Services Navigator will play a lead role in the development, implementation, coordination, and growth of a recent Transition Youth Program. Responsibilities include transition planning, system navigation, community partnership development, DSO application support, program development, public education, and community capacity building.
