Our model emphasizes continuity of care,



allowing nurses to develop long‐term relationships with patients and become a trusted part of their healthcare team.You'll work closely with physicians, registered nurses, physiotherapists, and rehabilitation staff to support patients with a wide range of health needs, from preventive care and chronic disease management to acute concerns that arise in the community.Most importantly, you'll have the time and support to deliver the kind of care that brought many nurses into the profession in the first place.What You'll DoConduct patient histories and nursing assessments within LPN scope of practiceMonitor and support patients with acute and chronic health conditionsPerform clinical procedures including wound care, injections, suture removal, and ear syringingAdminister vaccinations and prescribed medicationsMonitor nutrition, medications, and overall health statusPerform screening assessments and preventive care interventionsCollect specimens and complete point‐of‐care testingProvide education and guidance to patients and family caregiversCollaborate closely with physicians and other members of the interdisciplinary teamCoordinate care with community resources, specialists,
