MSP uses housing-first and harm reduction principles in the provision of a safe, respectful and accessible place for individuals who are experiencing homelessness or at risk of homelessness in the community. They work with individuals who access MSP services to identify, develop, implement and assess the self-identified goals of participants. Case managers work from a housing first, harm reduction and trauma informed lens to support people to find and maintain housing and to connect to primary care, income, addictions & mental health supports to break down barriers and advocate for their participants. Relationship building is the key function of this team.Referrals into this team come from support staff within MSP or external agencies. They are reviewed by the Program Manager and assigned to a case manager based on the space within a caseload, the acuity of the participant and their immediate needs. Once assigned, the case manager meets regularly with participants within MSP and the community.Key ResponsibilitiesGeneral Case Manager DutiesProvide on-going case management support to assess progress and ensure treatment planDevelop and coordinate individualized care plans addressing short-term and long-term goals in collaboration with participants, interdisciplinary team members and external partnersAssess an unsheltered person's needs in terms of moving towards housing Identify in a proactive manner, barriers that restrict and develop strategies that facilitate the participants' ability to meet identified goalsCreate and maintain a professional relationship with community members, including boundary settingKeep accurate, continuous, legible and signed records including progress notes, checklists, correspondence,



journals for individual and collective access in accordance with established protocols pertaining to access to and disclosure of casework filesWork with Involuntary Participants and Participants not capable of consentApply ethics of case management in all participant relationships including confidentiality, self -determination, informed consent and care taking the power differential between caseworker and participant.Planning process and intervention with participantIdentify and provide information and resources to participants including:Community resourcesRent and income assistanceMental health resourcesIndigenous organizationsEstablish partnerships with landlords and service agencies, and advocate on behalf of participantsIdentify symptoms of suicide ideation and offer resources and supportCreate environments of cultural safety for participantsMeet with participants both on-site and in communityMonitor, evaluate and communicate progress of participants with the participants themselves, team members and managers on a regular basisMaintain accurate and confidential electronic and file records, including statistical dataConduct crisis intervention as neededAttend program-related community, coalition, and committee meetingsSpecialized DutiesOutreachBuild effective relationships with encampment residentCoordinate with local agencies to provide services that meet the needs of residentsContribute to the development of a community that values cultural diversityPreferred experience with homeless outreach working with homeless individuals/families; proven ability to access the wide range of homeless services; familiarity with overcoming the barriers homeless people encounterHousing (The Bell)Provide supportive and proactive approach to eviction preventionProvide an open and welcoming environment for tenants to share their housing concerns and address tenants housing needsReview any client risks or issues and utilize team approach to identify action planExperience with Palliative and after death careExperience with Harm reduction in a supportive living environmentMMIWG2SProvide comprehensive case management services to families affected by MMIWG, demonstrating sensitivity to cultural nuances and the diverse needs of Two-Spirit individualsEstablish and maintain supportive relationships with families, acting as a liaison between them and relevant law enforcement agencies, social services, and other community resourcesFacilitate cultural ceremonies, healing circles, or support groups to address the emotional and spiritual needs of affected families and individualsCollaborate with local Indigenous communities, Elders, and cultural advisors to ensure that cultural perspectives are integrated into case management practicesContribute to the collection of data for research purposes to inform systemic changes and improve support servicesQualificationsBachelor's Degree in a relevant field. Experience working frontline with people who are unsheltered, lived experience and/or a combination of education and experience will be considered in lieu of a degree.Demonstrated knowledge of case management principles and processes – eligibility, assessment, planning, implementation, evaluating, transitioning, documentation and file managementDemonstrated experience developing and implementing care plans that promote positive health outcomesExperience creating and maintaining case plans, progress notes and other necessary documentationExperience working with Indigenous organization and/or communitiesExperience in working with culturally diverse populationsComputer proficiency in Microsoft Office and any other database software(s) used to track service deliveryNon-violent crisis intervention (NVCI), mental health first aid,



suicide intervention, and/or trauma informed training considered an assetKnowledge, Skills And AbilitiesStrong knowledge of MSP Van Outreach work and relationships with people living unsheltered and in Winnipeg encampments.Ability to apply to harm reduction approach and housing first principles to day-to-day workAn understanding of and ability to work within the frameworks of client-centred care, self-determination, strength-based approach, and trauma informed careExcellent interpersonal skills and the ability to form relationships with community members including a proven ability to communicate appropriate professional boundariesMotivational interviewing skillsExcellent verbal and written communication skillsKnowledge of Indigenous History and Colonial History and it's impact on the reality of Indigenous people in Winnipeg.Knowledge of TRC Calls to Action, MMIWG Call to Justice, and UNDRIPIndigenous language skills are considered an assetKnowledge of community resources available throughout ManitobaKnowledge of addictions, mental health diagnoses, and co-occurring disordersKnowledge and ability to adhere to confidentiality standards and legislationAbility to manage, assess, and work with challenging behavioursAbility to work independently, under pressure, in a team or independent settingAbility to effectively resolve and cope with conflict and/or crisis situationsStrong collaboration skills, both with team members and with external partnersAbility to receive and apply feedbackAbility to model qualified behaviours and demonstrate accountability with participantsAdditional RequirementSatisfactory Criminal Record Check with Clear Vulnerable Sector Search and a Clear Adult Abuse Registry Check are required.
