This role supports patients and families across our community through assessment, intervention, education, and collaboration with healthcare partners. The Occupational Therapist will support patients in achieving optimal function and independence in their daily lives through assessment, intervention, and education. As an integral member of the interdisciplinary team, the Occupational Therapist collaborates with patients, families, and healthcare providers to develop personalized care plans that address physical, cognitive, and psychosocial needs across the lifespan.Roles and Responsibilities:Primary Care IntegrationAttachment Efforts:Support patients who do not have primary care to be referred to the attachment team with the OCEAN e-referral.Access Efforts:Be an ambassador to the groups and workshops offered by HFHT by being aware of current offerings, handing out promotional materials and promoting the groups and workshops to existing, new and future patients.Contribute to improved access and continuity of care across local healthcare services.Quality & Performance ImprovementEnsure services reflect current best practices and ministry guidelinesLead or support program evaluation and quality improvement initiativesMonitor and evaluate outcomes to inform service planningPromote a safe, inclusive, and respectful care environmentAddress patient concerns with sensitivity and professionalismFoster a culture of patient engagement and shared decision-makingResource OptimizationSupport the effective use of resources to meet service demandsIdentify gaps and help develop processes that improve efficiency and capacityLeadershipModel the HFHT's values of collaboration, inclusivity,



and excellenceSupport and mentor team members to maximize team effectivenessChampion equity, diversity, and psychological safety in the workplaceServe as a leader, advocate and ambassador of occupational therapy in primary careOccupational Therapy Related Duties and Responsibilities:Conduct comprehensive assessments to identify physical, cognitive, and functional needs.Develop and implement individualized treatment plans to support daily living and independence.Provide education and strategies for areas such as managing chronic conditions, mental health, cognition, injury prevention, home safety, functional mobility and more.Collaborate with interprofessional team members to support coordinated, patient-centered care.Facilitate individual and group programs focused on wellness, self-management, and functional improvement.Recommend assistive devices, home/workplace modifications, and community resources.Document care and progress in the electronic medical record (EMR).Support quality improvement initiatives and contribute to program development.Assist patients with navigation of healthcare and community support systems.Qualifications and Skills:Required:Master's of Science in Occupational Therapy (MScOT) or equivalentRegistered and in good standing with the College of Occupational Therapists of OntarioUp to date professional liability insuranceDemonstrated competence across the full scope of occupational therapy practice, including assessment, intervention, goal setting, care planning, and documentation.Demonstrated clinical reasoning and therapeutic skillsAbility to work independently in a collaborative,
