About the Role:The Crisis Stabilization program is an eight-bed residential crisis service, providing an alternative to hospitalization or criminal detention for individuals experiencing a crisis related to their substance use and mental health. Staff provide de-escalation and stabilization of the immediate crisis and connection to the services needed to enable safe and stable community living that addresses client health and wellbeing longer term.The Support Worker will work as a member of an interdisciplinary team providing a variety of practical and emotional supports to facilitate crisis stabilization and effective community re-integration. Transportation of clients may be required; Evening/weekend work isrequired on a rotational basis.Hours of Work:Days (7:30am-5:30pm)Hours are scheduled on a 2-week rotation and includes working every second weekend. or social servicesMinimum one year of progressive experience in addictions or mental health; experience in concurrent disorders or trauma informed practice an assetExperience working with individuals experiencing mental health and/or substance use-related crisisExperience in social service settings with working knowledge of case management systems and planning techniquesLived experience related to addictions and/or mental health an assetCSC Clearance conducted by PWGS CanadaStandard First Aid/CPR Level CReliable Vehicle, Ontario Driver's license (G2 or G)Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) have declared scope of vehicle use to Insurance provider (overnight staff excluded)Willing to transport clients to appointments in your own vehicle (overnights excluded)Travel within the Wellington Guelph areaRole Overview:Client SupportSupport participants in achieving the wellness goals they have identified in relation to substance use, mental health, and other domains of lifeSupport plans for crisis intervention and risk managementSupport life skills development as identified in the clients personal goalsAssist with making referrals to internal and / or external programs or services, as appropriateAssist participants to connect to local resources by providing service navigation support and appointment accompaniment when necessarySupport the coordination of services with longer term supports,
