Job Summary

GEM nurses provide an expert clinical resource in hospital Emergency Departments. Their focus is on those frail seniors who are at risk of suffering adverse events, loss of independence and admission to hospital or long-term care. GEM nurses provide expert and targeted assessment and treatment services in the Emergency Department; they enhance linkage with hospital and community services and are advocates for the frail seniors they serve.

As well, in their role as geriatrics educators and capacity builders within the ED, the hospital and the community, GEM nurses help to build the systems capacity to ensure that frail seniors at risk receive the right care, in the right place, at the right time.

Responsibilities

- Provides a specialized clinical resource for assessing and treating the elderly (65+ years)
- Assist in the implementation of standardized protocols to screen and refer the high-risk elderly in the emergency department, including ongoing education and training.
- Provide appropriate assessments and consultation to identify acute symptoms, underlying health conditions, physical, functional, emotional and cognitive status, home environment issues and home supports needed for the at-risk elderly identified in the emergency department.
- Advocates for seniors to ensure that seniors at risk receive the right care, in the right place, at the right time
- Promotes a model of best practice in Elder Care and an elderly friendly philosophy of care within the Emergency Department
- Make recommendations for care for frail seniors and assist in their implementation within the emergency department, and other hospital services, including specialized geriatric services.




- Assist in the development of linkages with community services (e.g. family physician, Home and Community Care, long-term care settings, Community Support Services).
- Develop innovative solutions to address concerns with patient transfers between the ER and other community providers (may include patient information, education, development of protocols, etc.) with a view to building capacity in the system
- Educate patients/families/staff both formally and informally regarding the particular needs of the elderly and the specialized care they require in the ED.
- Provides expert and targeted assessment and treatment services
- Facilitates timely discharge of elderly patients from the Emergency Department through links to community resources
- Continues to advance knowledge of care of the elderly by creating a career development learning plan

Qualifications:
- Current RN Certificate of Registration from the College of Nurses of Ontario
- Baccalaureate degree in Nursing required
- Completion of education and/or training courses related to care of the geriatric patient/client/resident
- Completion of Masters Degree in Nursing, Education or Health Administration preferred
- Minimum 3 years recent experience in geriatrics and working with frail seniors and/or emergency services
- Gerontological Nursing Certification (preferred)
- Current member of RNAO.
- BCLS required

Salary Range: $41.15/hr - $58.98/hr
