The Care Coordinator position is responsible for all navigation of services for Actic clients and their plan members from assessment through to completing treatment plans. This includes call management, all on-going phone and email communication with Actic clients and their plan members and Actic healthcare providers for the purpose of coordinating quality services to all stakeholders.
The Care Coordinator will work together with Leadership and staff to create and maintain strong relationships between Actic and all stakeholders, clients, plan members, and all healthcare providers reinforcing the organizations reputation as a leader in digital assessment and treatment services.
This role reports to the Chief Operating Officer and collaborates with all Actic team members.
● Receive files from client case managers and start a corresponding Actic file
● Communicate with appropriate stakeholders to initiate the assessment process including plan member intake survey, coordinating an assessment with the appropriate healthcare provider and communicating next steps with the client.
● When a treatment plan is provided work with the client to implement the plan including conducting an intake interview with the plan member and working with Actic or other healthcare providers to ensure the plan is executed in a timely manner.
● Answer phone, respond to emails and provide appropriate and friendly services to our clients and providers
● Display masterful knowledge of Actic software, assessment and treatment processes and deliverables
● Provide first tier technical support to Actic platform users and escalate for more complex technical issues.
● When appropriate inform product development and testing for quality improvement.
● Collate data for mandatory client and internal reporting

Qualifications
● Post Secondary degree in a relevant field and 2-5 years experience in a customer relations and/or medical administration industry with demonstrated skills, experience and success in front line administration and customer service
