Your Opportunity:

The FAST Change Management Coordinator (CMC) is a key role reporting directly to the RAPID Manager within Primary Care Alberta (PCA). This position develops and maintains strong working relationships with internal and external partners, including Senior Consultants, FAST Operations, and Primary and Specialty Care offices, to ensure effective and safe quality care. The CMC works within the community physician/specialist space, supporting medical office staff in adopting and utilizing referral workflows. Additionally, the role facilitates the implementation of data-sharing processes between community offices and the FAST program. With a strong focus on the people side of change, the CMC helps integrate new practices related to referral management within the community setting.

Description:

The Change Management Coordinator for the FAST Program, supporting the implementation, adoption, and continuous improvement of referral processes across the healthcare system. Builds and maintains strong relationships with primary and specialty care providers, clinic staff, patient partners, and other stakeholders to promote engagement, collaboration, and change readiness. Provides virtual and in-person support to community clinics, facilitates the adoption of referral workflows, and prepares communication materials, meeting documentation, and reports to support program objectives. Coordinates referral data-sharing activities by working with physicians, clinic managers, medical office assistants, and community partners to collect, validate, and share data accurately and securely.



Identifies challenges, supports problem-solving efforts, and assists with data entry and reporting requirements. Supports training, onboarding, and knowledge translation activities related to electronic referral processes and systems. Identifies learning needs, assists with educational initiatives, and promotes effective use of referral tools and workflows. Contributes to quality improvement, patient safety, and quality assurance initiatives by gathering stakeholder feedback, supporting evaluation and reporting activities, participating in improvement cycles, and identifying opportunities to enhance program effectiveness and service delivery. Provides program and project coordination support through planning, monitoring deliverables, preparing presentations, reports, and templates, and supporting strategic and operational initiatives. Collaborates with internal and external partners to implement change management strategies, support stakeholder engagement, and advance FAST Program goals through effective communication, coordination, and continuous improvement.

Required Qualifications: Registered with the relevant college and associations. Post-secondary degree in a relevant field.

Additional Required Qualifications:

A minimum of three years in progressively responsible roles within a healthcare-based organization. Previous experience with a community Electronic Medical Record (EMR). Comfortable working independently and liaising between physician community offices and the FAST team.

Ability to maintain confidentiality to deal with patient information. Ability and willingness to travel within the designated region to community clinics, physician offices and partner sites. Strong communication skills and ability to navigate and resolve challenging conversations.

Proficient in the use of Microsoft Office especially Excel. This is a full-time on-site position requiring in-person collaboration.

Preferred Qualifications:

Previous change management and quality improvement experience and knowledge considered an asset (e.g. PROSCI, Alberta Improvement Way). Strong communication skills. Connect Care knowledge and experience is an asset. Display a teamwork approach, be self-directed. Comfortable working independently to liaise between physician community offices and the FAST team. Adaptable to competing priorities and flexible around physician/specialist schedules and availability.
