Our team works collaboratively to improve patient access, health outcomes, and system sustainability through innovative and integrated primary care services.

Position Summary The Manager, Chronic Disease & Population Health provides operational leadership for chronic disease services and community-based programming at Guelph FHT. Using a population health lens, this role leads service planning, program development, and implementation. The Manager identifies emerging community needs, advances health promotion, prevention, and early intervention strategies, builds partnerships that support integrated, patient-centered care, and provides leadership to interdisciplinary teams.

Key Responsibilities

- Provide operational leadership for chronic disease services and community programming, aligned with organizational priorities, community needs, and evidence-informed practice.
- Lead population health planning, community needs assessments, service development, program implementation, and initiatives that improve access, patient experience, and health outcomes.
- Identify emerging trends, service gaps, and opportunities for innovation, prevention, and early intervention.
- Lead performance management processes and support staff development through regular coaching, goal setting, feedback, and identification of growth opportunities.
- Establish and monitor performance indicators, targets, and quality measures,



and use data and evaluation findings to guide decisions, resource allocation, and service planning.
- Participate in quality improvement and change initiatives that strengthen care delivery, patient safety, integration, and sustainability.
- Oversee clinical and operational risk, patient safety practices, quality standards, privacy compliance, and related professional, ethical, and organizational requirements.
- Provide clinical oversight of medical directives, infection prevention and control, policies, procedures, and risk management activities.
- Ensure timely and accurate reporting to internal and external partners, including Ontario Health and other funding or accountability bodies.
- Support strategic partnerships with health care providers and community organizations
- Advance integrated care, health promotion, chronic disease prevention, early intervention, and community-based programming.
- Represent Guelph FHT in community and collaborative partnerships.
- Contribute to organizational planning, strategic decision-making, Annual Operating Plan activities, and management team initiatives.

Qualifications:

- Undergraduate degree in a regulated health profession, health sciences, public health, health administration, or a related field; a Master’s degree is considered an asset.
- Registration in good standing with a relevant regulatory body, where applicable.
- Minimum five (5)



years of clinical experience and five (5) years of progressive management experience in a health care environment.
- Demonstrated experience in population health planning, program development and evaluation, quality improvement, community engagement, and change leadership.
- Proven leadership in workforce planning, recruitment, performance management, and staff development.
- Experience developing and maintaining strategic partnerships across health care, community, and health system organizations.
- Solid knowledge of health care legislation, privacy requirements, clinical governance, and evidence-informed practice.
- Excellent leadership, communication, stakeholder engagement, and relationship-building skills.

Pay Range : $93,539-$99,236
