Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.With a staff of 500 members (including full time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.We are currently seeking a Registered Nurse (RN)/Registered Practical Nurse (RPN) to fill a vacant position at our Home Care Team.Terms : Part Time Permanent/CasualLocation: ScarboroughPay range : $33 – $37 hour lyResponsibilities:Provides client care needs assessment (including use of the InterRAI Cha), case management, crisis intervention, supervision and clinical field support for community health care providers delivering Home Care, and Assisted Living servicesActs as a clinical consultant for the Home Care team/department, Assisted Living, and other medically complex cases as requiredPrepare and submit Initial and Interim Client Assessment Reports to LHIN and/or other service partners as requiredAs a departmental and organizational team member, attend meetings and contribute to the planning, monitoring, reviewing, and evaluating of programs/services for continuous quality improvement,
