Position Summary

We are seeking a compassionate, reliable, and experienced Personal Support Worker (PSW) or In-Home Caregiver to provide dedicated, one-on-one care for a 74-year-old male living with advanced Parkinson’s disease.

The client has very limited mobility and communication abilities and lives in a private, fully accessible home equipped with an elevator, hospital bed, wheelchair, and other assistive equipment.

The successful candidate will provide attentive, respectful, and person-centred support with the client’s daily personal care, mobility, comfort, nutrition, and emotional well-being. The caregiver will work closely with the family and, as appropriate, members of the client’s healthcare team to ensure that care is provided safely, consistently, and in accordance with the established care plan and applicable Ontario requirements.

Key Responsibilities

Personal Care & Mobility

- Provide hands-on assistance with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, continence care, and personal hygiene.
- Safely assist with transfers, repositioning, and mobility using appropriate techniques, body mechanics, and available equipment.
- Assist with wheelchair use and movement throughout the home, including secure use of the elevator.
- Assist with positioning and comfort measures as required.
- Maintain a safe, clean, and fall-prevention-focused environment.

Health, Nutrition & Medication Support

- Provide medication reminders and, where permitted, medication assistance or administration in accordance with applicable Ontario requirements, the care plan, and employer direction.
- Observe and promptly report changes in the client’s physical condition, skin integrity, appetite, mood, comfort, or general well-being.
- Assist with meals, feeding, hydration, and texture-modified diets as required.
- Support prescribed exercises, mobility routines, or physiotherapy-related activities as directed by the appropriate healthcare professional.
- Follow established health, safety, infection-prevention, and emergency procedures.

Communication & Emotional Support





- Communicate patiently, respectfully, and attentively, adapting to the client’s limited verbal communication abilities.
- Develop an understanding of the client’s individual communication style and recognize non-verbal cues that may indicate discomfort, distress, or other needs.
- Provide companionship, reassurance, encouragement, and emotional support.
- Promote the client’s dignity, privacy, independence, and quality of life.

Household & Care-Related Duties

- Perform light housekeeping duties directly related to the client’s care, including laundry, linen changes, and cleaning and organizing care equipment.
- Prepare or assist with simple meals and snacks.
- Support adequate hydration throughout the day.
- Maintain a clean and organized care environment.
- Ensure medical and assistive equipment is used appropriately and kept clean and in good working order, reporting any concerns promptly.

Care Coordination & Documentation

- Follow established care plans, routines, and safety protocols.
- Maintain accurate daily care notes and document relevant observations.
- Communicate changes or concerns promptly to the family and/or designated care supervisor.
- Work cooperatively with family members, nurses, therapists, and other healthcare professionals as required.
- Maintain confidentiality and respect the client’s privacy at all times.

Required Qualifications & Experience

- PSW certificate from a recognized Ontario educational institution or equivalent relevant caregiving experience.
- Two years with previous experience caring for individuals with Parkinson’s disease, neurological conditions, or significant mobility limitations.
- Demonstrated knowledge and experience with safe patient-transfer and repositioning techniques.
- Experience using mobility aids and assistive equipment.
- Strong observation, communication, and interpersonal skills.




- Ability to perform the physical requirements of the position, including assisting with transfers and personal care.
- Ability to work independently while following established care plans and instructions.
- Legally eligible to work in Ontario.

Preferred Qualifications

- Current First Aid and CPR certification (Level C).
- Previous experience providing one-on-one care in a private home.
- Experience with advanced Parkinson’s disease or other progressive neurological conditions.
- Familiarity with Ontario home-care practices and standards.
- Vulnerable Sector Check, or willingness to obtain one.
- Additional relevant PSW, caregiving, or healthcare training.

Work Environment & Employment Terms

- Schedule: To be discussed. Day, evening, overnight, and/or live-in arrangements may be considered depending on the candidate and care requirements.
- Compensation: Competitive and commensurate with qualifications and experience, in accordance with applicable Ontario employment standards.
