Summary

This position combines responsibilities within our general nutrition program and some overlap into the Diabetes Care program. We are looking for a skilled and compassionate professional who is passionate about supporting individuals and groups in achieving their health goals through evidence-based nutrition care and diabetes education.

The Registered Dietitian will provide individualized nutrition counselling and medical nutrition therapy for patients managing chronic diseases (including Type 2 and Pre-Diabetes), digestive disorders as well as supporting seniors/aging population, infant feeding, eating disorders, weight management etc. The successful candidate will provide direct patient care in both individual and group settings within a Family Health team environment. Working collaboratively with Physicians, nurses and other allied health professionals, the Registered Dietitians will contribute to the delivery, evaluation, and ongoing development of nutrition services.

Job Type: 0.6 FTE Part-time permanent (3 days/week)

Work Location: in-person at either Rockwood or Erin clinics, with virtual opportunities

Requirements

Bachelor's degree in Food and Nutrition from an accredited university

Current certificate of competence as a Registered Dietitian with the College of Dietitians of Ontario





Certified Diabetes Educator (CDE) preferred

Experience providing group education and facilitating workshops/classes

Experience working in a primary care setting is considered an asset

Strong leadership, organizational, and problem-solving skills

Excellent verbal and written communication skills

Demonstrated ability to build positive and supportive relationships with patients, families, and healthcare professionals

Ability to work collaboratively with physicians and interdisciplinary team members, including Occupational Therapists, Mental Health Clinicians, Nursing Team and other allied health professionals

Proficiency with electronic medical records (EMRs); experience with Practice Solutions is an asset

Proficiency and experience working in a computerized environment

High degree of resourcefulness, flexibility, and adaptability.

Commitment to staff and patient privacy

Familiarity with East Wellington and the spectrum of health and social services available

Class G Ontario driver’s license, access to a car, and the ability and willingness to drive between clinics and, otherwise, as is occasionally necessary

Proof of up-to-date immunization, CPR/AED Level-HCP, N95 mask fit test, and vulnerable sector check
