The Caroline Family Health Team (C-FHT) is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our progressive team serving the residents of Halton on a Casual On-Call Basis to work every second Saturday clinic (7:45am - 1:00pm). This posting is not for an existing vacancy.

Responsibilities:

- Triage incoming patient calls
- Assess patient’s health needs, discuss test results and respond to questions pertaining to health promotion and disease prevention
- Redirect calls to booking when necessary. Notify Physicians of calls as per guidelines
- Implement Physician orders ensuring that patients undertake requisite diagnostic testing. Book appointments for patients and educate them as to the purpose and preparation for such tests
- Administer and record immunization
- Educate patients as to the need for and the possible side effects of such injections
- Treat patients with minor infections ensuring that appropriate testing and follow-up is undertaken
- Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
- Is guided by the standards and guidelines of the College of Nurses of Ontario in everyday work

Requirements and Specifications:





- Current certificate of Registration from your respective College
- Excellent patient assessment and triaging skills
- Knowledge of and working experience with EMR computer systems and applications
- Excellent verbal and written communication skills
- Ability to work collaboratively as a member of a multi-disciplinary team
