Position available: Saturday, Sunday (every other week) with shift pick up available

QUALIFICATIONS:

1. Current Certificate of Competence as a RN/RPN with the CNO.
2. Knowledge of nursing practices and nursing diagnoses.
3. Empathy for and understanding of the needs of the elderly/dementia clients.
4. Previous employment in a health care/hospitality setting an asset.
5. A minimum of 2 years of work experience in a geriatric setting.
6. Ability to relate to all levels of the residents and their families with a high degree of patience, tolerance and confidentiality.
7. Possesses initiative, good judgment and leadership abilities, with demonstrated supervisory capabilities.
8. Familiarity with the Unitarian House Mission Statement and values.
9. Willingness to update and keep current their knowledge of changing regulations, practices and procedures as required by the CNO and RHA.

JOB REQUIREMENTS:

1. Current CPR and FIRST AID Certification.
2. Police Record Check including VSC, as per RHA.
3. TB Testing.
4. Up-to-date Immunization as per UH policy

PHYSICAL DEMANDS:

1. Standing/sitting for long periods of time.
2. Walking short distances (along hallways) and up/down stairs.
3. Typing and/or use of a computer keyboard proficiently.
4. Pulling/pushing/lifting to relocate carts, equipment, and supplies up to 5 kg.
5. Reading/seeing documentation both in paper format and on computer screen.
6. Hearing differences in phone ring tones, fire system alarms or alerts for emergency response purposes.
7. Speaking/communicating to individuals with impairments and disabilities.
8. Bending/kneeling while performing resident care.
9. Dexterity required/ability to use a keyboard, open pouches/blister packs, insert test strips in glucometers, etc.

MENTAL EFFORT (DEMAND):



· Continuous mental effort is required for provision of safe resident care services.

· High probability of work interruptions and distractions.

WORKING CONDITIONS:

Active environment with designated work station and computer. Position requires good manual dexterity, and hand-eye coordination. Peak periods require an accelerated work pace and/or extended hours of work. Shared space with team members.

RESPONSIBILITIES:

1) Medication Management

· Ensure safekeeping of drugs and narcotic keys.

· Order/receive/store and safely dispose of Pharmacy medications.

2) Resident Care

· Contact resident’s attending physician when there is a change in physical or mental condition.

· Provide emergency medical care to residents in senior apartments.

· Answer call bells and assist PSW/UCP when necessary.

· Assist resident to obtain the services of a physician, as available.

· Assess for need and arrange shared-care services through OH@H.

· Organize OT/PT assessments as ordered (private/ OH@H).

· Arrange appointments for physician-ordered tests.

· Process physician orders; monitor resident trends (VS,Wt)

· Daily monitoring / assessment of health status of residents.

· Daily documentation of assessments/interventions.

· Manage emergency situations and call 911.

· Update resident care plan after health status change.

3) Supervisory / Quality Control





· Provide direction for and supervision of PSWs/UCPs.

· Adhere to UH processes and policies (UH,CNO, RHA) as related to care delivery.

· Assess PSW routines (toileting, bathing, housekeeping, tray delivery, laundry, night checks, cueing, escorting).

· Ensure maintenance of accurate records – narcotics, labs, medications, progress notes.

· Receive nursing report at beginning of shift/ Give report at end of shift.

1. Other

· Collaborate with volunteers and UH staff in the implementation of UH activity programs. · Collaborate with Dietary Services to optimize resident’s dining experience (seating, food allergies, special requirements).

· Perform all other job duties as assigned.

5) Risk Management and General Safety:

· Awareness of role and responsibilities under the OHSA.

· Practices safety procedures at all times, including Personal Protective Equipment (PPE), fire safety procedures; SDSs (Safety Data Sheets) and Lockout/Tagout procedures.

· Reports all accidents/incidents immediately.

· Reports all unsafe and hazardous conditions/equipment immediately.

· Participates in Fire Safety and Mock Disaster procedures.
