St. Leonard’s Place Peel (SLPP) is a community-based residential program that provides a place of safety, belonging, dignity, nourishment and education for homeless men with serious mental illness and those who have been in conflict with the law. We are looking for a compassionate, organized Care Coordinator who can combine solid case management with practical, client-centred support for individuals with complex mental health and addiction needs.

The Opportunity

Reporting to the Supervisor, the Care Coordinator coordinates care and provides case management support to clients with complex mental health and addiction needs. The role leads client assessments, develops and coordinates care plans, provides counselling and clinical support within the scope of the position, facilitates programs, maintains client documentation, and connects clients with appropriate community resources and services.

What You’ll Do

Client Care & Case Management

- Lead client screening and intake assessments and use appropriate tools to observe, record and report on individual functioning.
- Develop and maintain individualized care plans and provide ongoing case management in collaboration with clients.
- Provide assessments, counselling and interventions within the scope of the position and professional qualifications.
- Help clients navigate mental health, addiction and community service systems, and coordinate with the consulting psychiatrist, medical team and other service providers.
- Respond appropriately to crisis situations and identified client risks while maintaining professional boundaries.

Programming

- Develop and facilitate evidence-informed psychoeducation, therapeutic and capacity-building programs within the scope of the position.
- Develop and facilitate client education and support groups based on identified needs.
- Provide input and feedback on programs and work with the Supervisor to improve program delivery and client outcomes.

Documentation & Reporting

- Maintain accurate and timely electronic client records, case notes, reports, statistics and program documentation.




- Provide quantitative and qualitative reporting on program outcomes and impact.
- Maintain client information in accordance with privacy requirements and SLPP policies, and respond appropriately to inquiries while protecting confidentiality.
- Participate in case conferences, team meetings, town halls and required training, and complete other duties as assigned.

What You Bring

- Minimum Social Service Worker diploma or related education.
- Minimum 3 years of relevant experience working in mental health and addictions.
- Experience carrying a caseload, conducting assessments and developing and implementing individualized care plans.
- Experience responding to crisis situations and working as part of a case management support team.
- Experience developing and facilitating psychoeducation, therapeutic or client-support programs.
- Knowledge of serious and chronic mental illness, addictions, treatment approaches, community-based services and issues affecting individuals involved with the justice system.
- Working knowledge of the Mental Health Act, Substitute Decisions Act, 1992 and Health Care Consent Act, 1996, plus applicable privacy and confidentiality requirements.
- Experience using interventions within the scope of the position, including motivational interviewing, solution-focused approaches and cognitive behavioural approaches.
- Strong communication, facilitation, conflict-management, problem-solving, analytical and risk-assessment skills.
- Experience with database/client record management software and proficiency in Microsoft Office Suite.
- Current CPR/First Aid certificate or ability to obtain and maintain certification as required.
