Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery.

Wage Range: $113,153 – $141,441
About the Opportunity
We are seeking a candidate who shares our values and is committed to making a meaningful difference in the lives of seniors. This is an excellent opportunity for an experienced nursing professional who is passionate about resident care, quality improvement, leadership, and teamwork. Reporting directly to the Director of Long-Term Care Operations and Quality, the Nurse Clinician provides overall guidance, support, strategic leadership, oversight, and professional expertise to the Directors of Care and senior management team.
As a member of the corporate support team, the Nurse Clinician supports the development, implementation, and evaluation of clinical programs and ensures that all homes operate in accordance with the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, and all applicable legislation, regulations, and regulatory standards.
The Nurse Clinician also plays a key role in performance management by providing coaching, feedback, and disciplinary guidance as required to maintain high professional and ethical standards across resident care and service areas.
This role provides oversight, coaching, and support to RAI Coordinators and Quality Leads, and monitors the regulatory Continuous Quality Improvement (CQI) Program, Health Quality Ontario submissions, LTCF processes, Quality Improvement Plan development, and annual evaluations of required programs.
The Nurse Clinician leads teams in the development and implementation of specialized programs within resident care and services.
Key Responsibilities
Vision, Mission & Strategic Leadership
- Collaborate with the senior operations team in the development of the organization's and homes' mission, vision, and values.
- Work with Directors of Care on strategic planning and the establishment of annual goals and objectives.
- Work with ADOCs, RAI Coordinators, and the Quality Improvement team on quality indicators, improvement plans, annual evaluations, and Case Mix Index outcomes.
Governance & Leadership
- Provide guidance and direction to senior management in resident care, services, and legislative requirements, including the Fixing Long-Term Care Act, 2021 and Ontario Regulation 246/22.
- Lead the development of corporate policies and procedures across all areas.
- Assist homes in maintaining compliance with applicable statutes, regulations, and government requirements.
- Provide oversight, coaching, and support to Directors of Care, Associate Directors of Care, RAI Coordinators, and Quality Leads.
- Monitor RAI-LTCF outcomes and provide support for quality improvement initiatives.
- Provide guidance and participate in the development of nursing operating budgets and monitor the Case Mix Index for each home.
- Provide leadership in the development and implementation of special programs using an interdisciplinary and inter-facility approach.
- Provide guidance and support in the purchase of resident-related equipment and supplies.
- Complete and submit a written site-visit report to the Director of Care and Executive Director following each visit.
- Educate senior nursing management teams regarding new programs and legislative changes.
- Provide leadership, support, and guidance to Executive Directors in the absence of the Vice President of Operations and Director of Long-Term Care and Quality.
- Provide input to external committees and task forces regarding standards, programs, and initiatives.
Labour Relations & Human Resources
- Provide guidance and support to Directors of Care in interpreting collective agreements.
- Assist with the recruitment and hiring of Directors of Care, Associate Directors of Care, RAI Coordinators, Quality Improvement Managers, and other key staff.
- Provide input to performance reviews for Directors of Care, Associate Directors of Care, RAI Coordinators, and Quality Improvement Managers.
- As a member of the senior management team, participate in recruitment, selection, training, and performance management of in-home staff.
- Participate in the analysis of management information and implementation of related measures.
- Assist with the orientation of new Executive Directors, Directors of Care, Associate Directors of Care, RAI Coordinators, Nurse Clinicians, and Quality Leads.
- Assist in the development and approval of Human Resources policies and procedures based on current employment practices and applicable legislation.
Communication & Professional Relations
- Participate in monthly Joint Directors of Care meetings.
- Co-facilitate the Quality Council.
- Co-facilitate the Joint RAI Committee.
- Attend Continuous Quality Improvement Committee meetings at the home level at least twice annually.
- Lead assigned joint committees as required.
- Communicate high-risk issues to the Director of Long-Term Care and Quality.
- Communicate RAI or home-specific quality concerns to the Director of Care and/or Executive Director.
- Meet regularly with home staff to provide guidance regarding compliance, risk management, quality improvement, RAI-related matters, and resident care issues.
Fiscal Resources
- Provide guidance and direction in the development of nursing operating budgets.
- Monitor budget variance reports and assist Directors of Care with budgetary control measures throughout the year.
- Provide guidance and support regarding new funding initiatives and appropriate allocation of funds within each home.
- Monitor Case Mix Index and RUG-Weighted Patient Days.
- Review documentation and operational processes and make recommendations to homes regarding improvements to systems and practices.
Quality & Risk Management
- Co-facilitate Quality Council and Joint RAI Committee meetings.
- Provide oversight of the Continuous Quality Improvement Program within resident care services and provide feedback to Executive Directors, RAI Coordinators, Quality Leads, and Directors of Care.
- Monitor Health Quality Ontario submissions.
- Provide oversight of the RAI-LTCF process within the homes, including completion of Relias proficiency testing.
- Conduct data-quality audits of LTCF documentation and care plans as required.
- Conduct random audits and quality improvement activities in each home to ensure best practices are followed and Ministry of Long-Term Care and other legislative standards are met.
- Lead and actively participate in the annual corporate Resident Quality Inspection (RQI) of each home's programs and services.
- Provide written feedback following each RQI and assist homes with the development and implementation of action plans to address identified gaps.
- Conduct follow-up reviews following the annual RQI to ensure implementation plans are being followed.
- Provide guidance and support in responding to Ministry of Long-Term Care inspection reports and other required documentation.
- Research evidence-based quality improvement practices and recommend organizational changes to internal quality improvement systems as required.
- Maintain an understanding of the nature and meaning of quality indicators used by the homes, including indicators that are publicly reported.
- Support the improvement of quality indicators and achievement of satisfactory outcomes.
- Participate in the development of annual Quality Improvement Plans.
- Support the annual evaluation process in collaboration with the interdisciplinary team.

- Identify potential liability issues and collaborate with senior staff to minimize organizational and resident risk.

- Take responsibility for ongoing professional development and continuing education.
- Educate staff on Ministry of Long-Term Care standards, continuous quality improvement, RAI-LTCF processes, new policies and procedures, and other relevant topics.
- Complete all required training in accordance with the Fixing Long-Term Care Act, 2021 and Ontario Regulation 246/22.
Qualifications
The successful candidate will possess:
- Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN).
- Advanced education or courses in gerontology and health care services administration is preferred.
- AIS Certificate and sound knowledge of the RAI-LTCF process.
- Proven management experience in long-term care.
- Proven experience in facility management.
- Strong written and verbal communication skills.
- Knowledge of nursing operational budgets.
- Sound knowledge of the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, College of Nurses of Ontario standards, and other applicable legislation.
- Training in Quality Improvement.
- A positive and collaborative working attitude.
- Completion of a long-term care home administration or management program of at least 100 hours is considered an asset.
- Exceptional team-building, mentoring, and coaching skills.
