OverviewPharmacist, Community Pharmacy Services role at Fraser Health Authority. Casual opportunities for Pharmacists to join our team at Crosstown Clinic, Downtown Community Health Centre and Hope to Health Centre in Vancouver, BC. Community pharmacy experience or interest, including work with marginalized Mental Health & Substance Use populations, is required.ResponsibilitiesInterprets medication orders/prescriptions and dispenses medications using a computerized medication information system in accordance with legal, skilled, and departmental policies, procedures and standards.Assesses prescriptions for therapeutic suitability by evaluating the client's health status and needs, referencing primary drug information texts, interpreting laboratory data, and reviewing client demographics as needed.Reviews client profiles for allergies, potential drug interactions, therapeutic duplications or contraindications; intervenes as appropriate to ensure safety.Communicates with physicians, clients, and other health care team members for prescription clarification and problem resolution; documents outcomes of interactions.Provides instructions on proper use/administration, storage and disposal, potential side effects, and drug information to clients and professionals; maintains medication records and performs counseling.Assists with medication histories, reconciliation, and verification of allergy information; supports safe,



accuratedispensing and labeling processes.Monitors and reviews drug therapy through basic drug monitoring, dose recommendations for high-risk medications, and participation in medication use evaluation projects.Participates in dispensary-based medication safety initiatives, quality control activities, and related documentation; contributes to internalpublications and clinical drug trials where applicable.Acts as a clinical pharmacist to resolve select drug therapy problems; participates in rounds or health care team meetings and documents therapeutic plans.Performs other related duties as assigned.QualificationsEducation and ExperienceCurrent registration as a Pharmacist with the College of Pharmacists of British Columbia.Skills and AbilitiesKnowledge of drug distribution processes in a community setting; effective communicationskills (verbal, written, non-verbal); empathy and trauma-informed practice; professional judgement within legislative requirements; ability to work collaboratively and prioritize work; ability to operate related equipment and software; physical capability to perform duties.About Fraser HealthFraser Health is a large health system serving Metro Vancouver and the Fraser Valley.
