AttachmentTemporary 0.5-1.0 FTE Employees Calgary, AB, CAEmployment Type: Temporary until March 2027FTE: 1.0 FTE (40 hours per week)Work Schedule: Weekdays - DayshiftSummary of PositionThe Coordinated Attachment program's mandate is to support unattached patients, with chronic complex conditions living within Calgary area, by creating successful patient attachment in a specialized medical home. The program comprises family physicians, behavioural health consultants and nurse practitioners. Currently, this program accepts referrals directly from the hospital discharge referral pathway.Reporting to the Program manager, the Licensed Practical Nurse (LPN) works collaboratively with all clinic staff, multidisciplinary team and physicians in support of patient care. The LPN compassionately provides routine care and patient health assessments, assists doctors and other clinic staff, communicates instructions to patients regarding medication, home-based care, and preventative lifestyle changes and other clinical or administrative tasks.Key ResponsibilitiesKey aspects of the position may include, BUT are not limited to:Completing full patient assessments and triaging including:greeting and rooming the patientobtaining and recording vital signs, height, weight and blood pressure when appropriaterecording a brief and concise history relevant to the patient's chief complaintobtaining and processing urine samples when appropriateReviewing patient charts daily for completeness, ensuring all pertinent results are on chart and the chart is signed by the practitionerChaperoning all physicians during pelvic and breast exam when needed and appropriateProcessing specimens for transportUsing critical thinking, make decisions based on analysis of available informationPerforming nursing interventions for which they can manage the client during and after the intervention or have access to resources (O2 administration, dressing changes, injections,
