It focuses on helping them achieve housing stability and improve their quality of life by providing access to housing, practical assistance, counseling, advocacy, landlord mediation, and eviction prevention.As a member of the Allied Health Team, the PSH RN works with vulnerable individuals who are residents at the permanent supportive housing buildings at 10 Shelldale, as well as Grace Gardens, and the transitional housing program at 65 Delhi Street. The RN will provide independent top-of-scope medical care.



They will work as part of an interdisciplinary primary and allied healthcare team to provide health care and wraparound services for residents and clients. This position works at multiple sites in the community.Principal Job Duties:Clinical:Collaboratively provide care for clients experiencing significant trauma and who may be taking multiple medications for mental health and chronic health conditionsScreen, monitor and assess patients using appropriate depression, anxiety, chronic pain, addictions and chronic disease management screening and assessment toolsComplete in-person and virtual primary care visits, according to scope and college guidelinesLiaise with physicians, nurse practitioners and other allied health professionals and community partners to collaborate about client careWork at various locations, including at permanent supportive housing buildings, Guelph CHC sites and in community to provide health care to the priority populationOrganizational:Participate in all relevant CHC programs, activities, meetings, committees, workshops, training and on-going professional developmentCollaborate with interdisciplinary teams to identify and provide health education priority populations and staff working with priority populationsAssist in the development, monitoring, and analysis of continuous quality improvement and clinical initiativesAdhere to the Guelph CHC Confidentiality Policy,
