The successful candidate will help to support client medical management and work across the Champlain Region as a member of an interprofessional team using a palliative care approach.If you are passionate about what you do, motivated by improving the health of the seniors in our community, and committed to excellence, quality, and patient safety, we would like to hear from you.Key ResponsibilitiesAble to conduct comprehensive geriatric assessments to design individualized plans of careRecent clinical experience working with seniors with mental health and substance use concernsStrong skills in clinical assessment, screening, illness prevention,



and disease management to improve health outcomes and facilitate access to health servicesExperience with Advanced Care planningAble to have reliable illness conversationsAptitude and interest in client education and health promotion with seniorsKnowledge of community resources for seniorsAble to work independently as well as part of a teamRecent experience working in a community-based setting and/or ability to work independently in the communityQualificationsRegistered with the College of Nurses of Ontario, with no restrictions to practiceCertificate of Basic Life Support (BLS) for Healthcare Provider or equivalent such as Cardio-pulmonary Resuscitation/Automated External Defibrillator (CPR/AED) Level CValid driver's license, insurance, and use of a personally owned vehicleFluency in English and FrenchProficiency in the use of computers, software applications, and electronic documentationVulnerable sector screening completed within the last 12 monthsGerontological nursing certification and/or training is an assetSalary$73,736.00 - $86,750.00 per year depending on experience (HOOPP eligible)Additional InformationFor more information on our Centre visit www.Seochc.On.Ca.Artificial Intelligence will not be used to screen, assess or select applicants for this position.
