The work is delivered by inter‐professional teams that address both clinical issues and social determinants of health within a recovery‐oriented framework.Overview of the roleThe Nurse Practitioner offers comprehensive primary care to individuals, families and the community with a strong emphasis on health promotion, patient education and disease prevention.AssessmentWorking within the scope of practice of the RN (EC) to perform comprehensive, symptom‐directed assessments; synthesizing data from multiple sources to diagnose health problems; identifying lifestyle, psychological, sociocultural, and environmental risk factors; coordinating care through referral or consultation with other health providers and agencies.TreatmentProviding nursing care and treatment that may include pharmacological, complementary, or counseling interventions; prescribing scheduled drugs within the RN(EC) scope; collaborating with physicians and allied health to develop individualized care plans; monitoring chronic illness; and implementing strategies to promote independent,
