The RN collaborates with an interprofessional team to manage an assigned caseload, including assessments, coaching, interventions, client care, and follow‐up. The role focuses on health promotion, chronic disease management, and coordination across the healthcare continuum.ResponsibilitiesEstablish therapeutic relationships with clients, ensuring autonomy and privacy.Screen referrals, provide individualized assessments, and coordinate interprofessional care planning.Develop comprehensive shared patient care plans in collaboration with team and clients.Provide direct client care as needed and identify additional care services in line with guidelines.Delegate and supervise community health workers on client‐specific tasks, ensuring training and compliance.Facilitate care conferences, review plans,
