Temporary Full-Time (12-Month Contract)
Pay Rate: $115,156 - $137,503 Annually The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate patient flow through the health care system and ensure recovery for those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St), Harbour Lights, and a Specialized Care Centre (Victoria Park & McNicoll.
We are seeking a collaborative professional for a temporary full-time opportunity to contribute to our purpose and values – the Manager, Program Operations and Performance - who is passionate about making a meaningful difference in the lives of patients and families through integrated outpatient health care services. The outpatient clinic serves 10-15,000 visits per year and currently encompasses rheumatology, respirology, geriatrics, penicillin allergy testing, psychotherapy, and spasticity, and is served by 15 physicians.
This position is located at our Overlea site and is a temporary full-time position for a period of approximately 12 months.
In the coming year, we anticipate expanding to a satellite site in addition to our main site at 650 Church Street, Toronto.
Position Summary:
The Director of Integrated Transitional Care at the Salvation Army, Toronto Grace Health Centre, is currently seeking a Manager, Program Operations and Performance for the Remote Care Monitoring (RCM) program. RCM is a technology-enabled program that supports older adults to live safely and independently at home by providing safety devices, 24/7 monitoring, and access to support when needed. The program serves clients across multiple communities and works collaboratively with healthcare and community partners to support aging in place.
The Manager, Program Operations and Performance provides operational and staffing leadership for the RCM program across Ontario and Prince Edward Island. The position is responsible for day-to-day operations, program performance, and ongoing quality improvement.
Working under the strategic direction of the Director, the Manager uses program data to identify issues, plan for changing service needs, and help ensure program resources are used effectively.
The Manager also provides direct leadership to RCM staff and is responsible for staff performance and the consistent execution of established organizational and program procedures.

Responsibilities:
Business Operations
- Oversee the day-to-day business operations of RCM across Ontario and Prince Edward Island, including regular communications with program partners.
- Review and analyze service volumes, workload, and staffing levels to identify capacity needs and support effective resource planning.
- Develop operational forecasts to assess how changes in service volumes, funding, or program requirements may affect program operations.
- Oversee device inventory processes, ensuring appropriate tracking and controls are in place.
- Support the implementation of operational plans and priorities.
Performance & Analytics
- Monitor and report on key performance indicators, service levels, and other measures of program performance.
- Monitor program performance against established expectations and ensure appropriate action is taken where gaps are identified.
- Prepare program performance reports to support planning and management decisions.
People Leadership
- Provide direct management and supervision of RCM staff, including onboarding, performance reviews, coaching, employee development, and performance management.
- Establish clear roles and expectations, support effective delegation, and ensure accountability.
- Foster a collaborative, respectful, and high-performing workplace culture.
Quality & Process Improvement
- Identify recurring or systemic issues affecting program performance or service delivery and develop practical solutions.
- Lead process-improvement initiatives to strengthen workflows and client experience.
- Ensure program policies, procedures, and escalation processes are in place and followed.
- Support the implementation and monitoring of changes to program policies and practices.
Minimum Qualifications:
- Master’s degree in Health Administration, Healthcare Management, Business Administration, a regulated health profession, or a related discipline required.
- Minimum three (3) years of management experience in healthcare or a similarly complex service-delivery environment, including direct responsibility for staff.
- Demonstrated experience in operational planning, performance analysis/review, and resource management.
- Demonstrated experience applying business and process improvement approaches, such as Lean principles, to improve workflows and make effective use of program resources.
- Strong knowledge of the healthcare system and publicly funded healthcare environments, particularly home care.
- Experience in digital health, remote patient monitoring, home care or related services is an asset.
- Experience supporting multi-jurisdictional programs is an asset.
- Additional training or certification in healthcare leadership or project management is an asset.
- Strong business acumen, with the ability to understand budgets and service demands.
- Strong people leadership skills, including delegation, coaching, performance management, conflict resolution and employee development.
- Strong analytical and problem-solving skills, with the ability to interpret program data, identify issues, and recommend practical solutions.
- Demonstrated ability to manage competing priorities and maintain oversight across multiple operational areas.
- Excellent interpersonal, written and verbal communication skills, with the ability to work effectively with staff, senior leaders, healthcare partners, vendors and other stakeholders.
- Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and Teams.
