Job Summary

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our energetic healthcare team. Responsibilities
Deliver comprehensive patient care
Administer medications accurately following physician orders and ensure proper documentation using electronic health record systems
Perform clinical procedures including phlebotomy, pain management interventions, and wound care
Monitor patient vital signs, assess symptoms, and respond promptly to changes in condition
Assist with activities of daily living for patients with dementia ensuring dignity and comfort
Collaborate with physicians, case managers, and other healthcare specialists to develop and implement individualized care plans




Maintain detailed medical records and ensure compliance with healthcare regulations and safety standards
Provide education to patients and their families on health management, medication use, and post-discharge care

Experience
Proven experience working in nursing homes or hospital settings with a focus on acute care or senior care
Hands-on experience in dementia care, hospice support, is highly desirable
Skilled in medication administration, medical terminology, and laboratory procedures such as phlebotomy
Familiarity with electronic health record systems including Epic and Cerner is preferred
Background in home care or assisted living environments enhances your ability to deliver personalized patient-centered services
