Employment Type: Temporary until March 2027FTE: 1.0 FTE (40hours per week)Work Schedule: Weekdays - DayshiftSummary of PositionCalgary Foothills Primary Care Network (PCN) is a group of family physicians and health care professionals in northwest Calgary and Cochrane collaborating with various external stakeholders to deliver the best possible primary care. Calgary Foothills PCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making.The PCN Health Team is comprised of registered nurses, clinical pharmacists, social workers, dietitians and behavioural health consultants. Team members are co-located in member Patient Medical Home locations, centralized in clinic sites and deliver care in individual and group formats.Reporting to the Program Manager, the Registered Dietitian will work collaboratively with physicians, community partners and interdisciplinary team members to support patients who require nutrition advice related to chronic and complex health conditions.Key ResponsibilitiesKey aspects of the position may include, but are not limited to:Works out of clinical settings as a part of a multidisciplinary teamProvides nutrition care through individual and group appointmentsDetermines nutrition priorities through clinical assessment and develops patient centered nutrition care plansProvides patient centered nutrition education and counsellingActs as a nutrition resource for Calgary Foothills PCNProvides evidence-based nutrition information to patients, health team members,
physicians and community partnersMay participate in various stages of nutrition program planningMay be involved in quality improvement projects or health initiatives involving community partners and/or Calgary Foothills PCN Health Team members and physiciansCollects and maintains data for service indicators, progress reports and evaluationsKnowledge, Skills & AbilitiesExcellent communication skills including active listening, motivational interviewing and group facilitationDemonstrates initiative, accountability, good time management and organizational skillsAbility to work collaboratively and cooperatively in a team settingAbility to apply knowledge, skills and clinical judgement required for independent practiceAbility to work flexible hours, including evenings/weekends, when requiredAccepts, implements and evaluates change with a positive attitudeAbility to thrive in a agile and evolving environmentGood computer skills are requiredQualificationsBachelor of Science with a major in Foods and Nutrition requiredCompletion of Dietetic internshipActive or eligible for registration with the College of Dietitians of Alberta (CDA)Minimum three years' experience preferredExperience in primary care will be considered an assetExperience facilitating group sessions considered an assetEmployment Requirements:Completion of a satisfactory criminal record check and/or Vulnerable Sector Search.Diversity & InclusionOur PCN strongly believes in sustaining an inclusive, respectful and equitable working environment that represents the communities we serve.
