We provide community-based, mental health, and developmental services across Northwestern Ontario in partnership with families, schools, child care centres, and community organizations.About the RoleCoordinated Service Planning is designed to provide children and youth with multiple and/or complex special needs, and their families, with a seamless, family-centred service experience. As a Service Coordinator, you will lead inter-agency, multi-disciplinary care teams to develop and implement a single, integrated Coordinated Service Plan that reflects the child and family's strengths, vision, and goals. You will play a central role in ensuring services are connected, collaborative, and responsive as children grow and their needs evolve.Reporting to the Clinical Manager, you will:Engage children, youth, and families to identify their vision, strengths, and goals.Develop individualized, strength-based Coordinated Service Plans.Facilitate inter-agency, multi-disciplinary team meetings.Lead collaborative planning processes to ensure seamless service delivery.Connect families with appropriate local, regional, and provincial services.Monitor progress and adjust service intensity based on changing needs.Apply standardized screening tools.Support conflict resolution and guide teams through competing priorities.Initiate and facilitate complex special needs process when needs exceed locally available resources.Contribute to community development by participating in and leading community networks supporting children with special needs.Maintain a high standard of qualified conduct and quality of care.This role requires strong leadership, maturity,
