Conduct detailed in-home assessments and mobility evaluations
Design personalized, goal-oriented treatment strategies
Monitor client progress and adapt care plans as necessary




Coordinate with healthcare providers for comprehensive support
Educate clients about safety and independence strategies Requirements:
Registered in Ontario with the College of Occupational Therapists
Experience in community/home care preferred
Proven skills in assessments and treatment planning
Ability to manage documentation effectively
