Reporting to the Executive Director, this position supports two facilities on a biweekly basis, focusing on ethical practices and regulatory compliance. Key Responsibilities:
Coordinate care conferences for resident admissions
Deliver educational programs on social work topics
Provide palliative support and family advocacy
Collaborate within the interdisciplinary care team
Connect residents to community services and resources Requirements:
BSW/MSW or SSW with registration in valuable standing
Experience in healthcare or gerontology
Excellent interpersonal and organizational skills
