Job Summary

Join our dynamic healthcare team as a Registered Nurse (RN) specializing in Home Health Assessments! In this vital role, you will deliver comprehensive nursing evaluations and personalized care plans directly in patients’ homes, ensuring optimal health outcomes and enhanced quality of life. Responsibilities:
Conduct in-home client assessments to determine care needs and eligibility
Develop and document individualized care plans based on assessment findings
Communicate assessment results and recommendations to the Sterling Health care team
Coordinate with home support staff to ensure care plans are implemented appropriately
Maintain accurate, timely clinical documentation

Qualifications:
Active registration with CARNA (College of Registered Nurses of Alberta)




Minimum 2 years of community/home care nursing experience
Comfortable conducting independent, in-home assessments
Strong clinical judgment and documentation skills
Valid driver's license and reliable vehicle (travel within the community required)
Current CPR/First Aid certification

Pay: $50.00-$60.00 per hour
