The Geriatric Emergency Management NP (GEM-NP), will provide specialized care to emergency patients with geriatric syndromes and frailty with moderate acuity and high complexity needs. As part of a physician-led team, the NP will conduct geriatric assessments, provide interventions, and develop care plans.The NP will collaborate extensively with the interdisciplinary team, community resources and patient caregivers to support discharges, and they will be responsible for following up patient status in the community. We are seeking Nurse Practitioners who have geriatric expertise that can adapt to the energetic emergency environment.
