The Ottawa Hospital is seeking a Registered Nurse to join the Transitional Care Unit (TCU) on our General Campus. You will care for medically complex patients transitioning from hospital to home or another setting, in a 34-bed unit with a robust emphasis on collaboration and patient-centered care.
The role offers days and nights shifts, including weekends, with a focus on guiding patients and families through the transition and discharge planning to minimize readmissions.
